Medical Record Authorization

This form gives us permission to request and review your past OB and medical records. It allows our team, and eventually the fertility clinic, to confirm that surrogacy can be pursued safely based on your health history.
We only request records that are necessary, and this information is kept strictly confidential. By completing this form, you’re helping us ensure that your journey begins with the highest level of care, safety, and informed support.

This form helps us review your current health insurance to determine whether it’s surrogate friendly and what coverage may apply during your journey. Providing this information early allows our team to identify any gaps, ensure compliance, and plan the safest, most supportive path forward for you.

This form outlines the key expectations and responsibilities of being a Surrogate so you know exactly what the journey involves before moving forward. It’s meant to protect you, create clarity, and ensure there are no surprises along the way. By reviewing and acknowledging it, you’re confirming that you understand the commitments involved and that you feel ready, supported, and informed as you take your next steps.
The Agency Agreement is the foundation of our partnership together. It explains how we support you, what you can expect from us, and the promises we make to you throughout your journey. Its purpose is to create transparency, trust, and a shared sense of security so you feel cared for, protected, and never alone as you step into this meaningful role.
Your custom compensation package is crafted around you, your needs, your family, and your life. No two Surrogates are the same, and your compensation shouldn’t be either. This step ensures you feel valued, respected, and supported in a way that honors the generosity of what you’re choosing to do. Your time, your effort, and your heart deserve to be acknowledged in a way that feels meaningful and personal. This is where we listen closely, advocate for you, and build a package that reflects your worth.
During your virtual home visit with Ashlee, you’ll have the chance to connect beyond the paperwork to share your story, your hopes, and what you want this experience to feel like.
Ashlee will walk you through the process with honesty, warmth, and deep respect for the role you’re stepping into. The home visit isn’t an inspection; it’s a chance to meet you in your space, understand your support system, and ensure you feel seen, valued, and supported from the very start. These moments are where the heart of your journey truly begins.
Creating your Surrogate profile is a meaningful step in sharing your story with Intended Parents. Together, we’ll gather the details that reflect who you are like your personality, values, lifestyle, support system, and the heart behind your decision to become a surrogate. We thoughtfully craft this into a beautiful, authentic profile that helps intended parents feel connected to you from the very start.
This step is important because it sets the foundation for a strong, aligned match. When your profile feels true to who you are, it allows intended parents to see the real you, your warmth, your motivations, and the intention you bring to this journey so the connection begins with trust, understanding, and genuine compatibility.

These are items that will be needed, but can be done at any point during the onboarding process.

Candidates who are ready to begin their journey must have had a recent pap with negative/clear results. If you haven’t gotten a pap in the last 3 years, one is required in order to move forward in the process. These will be in your medical records if it has been performed recently.

We require a signed form from your current OBGYN/Midwife stating that he/she has no concerns if you become pregnant again, and that you have his/her support to be a surrogate. We will request this for you, along with your medical records. You may also have it filled out if you are going in for a pap smear at the same time. You can download our form for your OBGYN/Midwife to fill out if you would like.

Obviously if you are pursuing surrogacy, we do not want to be faced with an unexpected pregnancy. Please ensure that you are on reliable and consistent birth control. Natural family planning, or condoms, are not considered reliable. Do not use an implant, shot or IUD. If you have had any negative side effects or issues with certain birth control pills, please let us know as well. If you are not currently on birth control pills, you may be asked to discuss starting them with your OBGYN.
If you have an IUD, it will need to be removed and out for a minimum of 2 cycles (3 cycles for Mirena) before you can be cleared for embryo transfer, as it can negatively affect uterine lining. If your cycles do not start after IUD removal, you should follow up with your OBGYN for an evaluation. FMS recommends having IUD removed after medical records have been approved by our agency in order to avoid unnecessary removal if you are not approved.
While annual visits with pap should be covered as a preventative care exam if you have insurance, IUD removal is not. Any costs associated with obtaining medical clearance from your OBGYN, IUD removal, and any replacement birth control prescribed are your financial responsibility.

Surrogates at any point in their journey, from approval through postpartum, are invited to join our private support group on Facebook! You will find lots of great advice, and meet women who are at the same stage in their journey as you. Once you request to join, an admin will approve you.
